These packages are designed with you in mind.  Our nurses & care team will consult with you to design your package around your needs and wants. 

Initial Consultation: $300

Our nurses and team will meet with you virtually for the initial consultation. We will create your custom plan to be delivered to you! 

Includes:

• Nutrition
• Physical activity plans
• Customized health goals

Full Package: $1200

Our full package includes an initial consultation, followed by creating your custom wellness plan.  Your wellness team will be available for weekly check-ins to assess and assist with goal optimization. We also arrange a virtual meeting on a personalized schedule up to 6 meetings.

• Optimized custom health management plan 
• Weekly checkin with care team 
• Virtual meetings with care team
